Progressive multifocal leukoencephalopathy is a rare bsAb-associated complication
According to a report published in Blood Advances, the odds of reporting progressive multifocal leukoencephalopathy were higher with bispecific antibody therapy than with other myeloma therapies. The findings highlight the importance of prompt assessment for this complication in bsAb-recipients who develop subacute neurological symptoms, the authors noted.

Persistent T-cell activation detected in teclistamab nonresponders
Analyses of bone marrow and peripheral blood samples from patients enrolled in the MajesTEC-1 trial of teclistamab in heavily pretreated patients with RRMM revealed sustained T-cell activation, indicated by persistent higher expression of checkpoint markers on CD4+/CD8+ T-cells. The analysis, published in Haematologica, also showed an increase in immunosuppressive regulatory T-cells (Tregs), which may underpin resistance to immune-based therapies.

Rapid response kinetics prognosticates inferior outcomes in patients with MM
Slow responders — who had higher-than-median values for time to first response and time to best response — had significantly better PFS and overall survival than rapid responders, according to a retrospective study of patients with RRMM who received carfilzomib- and ixazomib-based regimens. Gaps in MM diagnostic testing identified in US primary care practice
Testing for plasma cell disorders is underutilized in US primary care practice, with 46% of respondents not ordering diagnostic testing, according to a survey-based analysis reported in The Journal of Applied Laboratory Medicine. Even when ordered, guideline-adherent paired testing of serum protein electrophoresis and serum free light chains was suboptimal, used by only 28%.
